This is a limited services clinic intended to provide basic services to healthy pets.  No prescriptions will be written (including flea and heartworm prevention).

Clinic is open to all residents of Ashtabula County, and nearby communities (e.g. Madison )

A non-refundable payment of $15 per pet (towards your total) is required at time of registration. If you cancel your appointment for any reason, you will not receive a refund.

You can register a maximum of 4 pets.

Payment - in cash - at time of service (see fees in the table below).

You will be asked for your preferred time slots. We will send you a follow-up email with your appointment time.

As part of registration, we are asking you to choose what types of services you wish to get - note, this is only for planning purposes. It's OK if you change your mind and want to add/remove services. You will make the final decision at check-in on the day of the clinic.

* For dogs: Nobivac 1-DAPPv is a core vaccine for protection against canine distemper virus, adenovirus type 1 and 2, canine parainfluenza virus, and canine parvovirus. For cats: Nobivac 1-HCP is a core vaccine for protection against feline rhinotracheitis, calici, and panleukopenia viruses.
** selamectin (brand name Revolution) is used as flea treatment in dogs and cats. This product also treats intestinal parasites in cats (except tapeworm), and ear mites in both dogs and cats. It is also a heartworm preventative.
*** a microchip is the size of a grain of rice, inserted between your pet's shoulder blades. It contains an ID that can be read by a microchip scanner (vy vet clinics, shelters and dog wardens). The ID will be registered with your name and phone number, so if your pet gets lost, you can be notified. There will be no additional cost i.e. you can update your pet's microchip account with new information (phone, address) as many times as you want without any charge. The Animal Welfare Center will do the initial registration for you (you will receive an email with a link to your account).
