HOLE SPONSORSHIP FORM

$ 50 - Hole Sponsor: your sign will be displayed at a hole during the game.

$100 - Ball Drop Hole Sponsor: your sign will be displayed at a hole during the game, then moved to the Ball Drop Hole. Your sponsorship will be verbally announced by the Ball Drop host.

If you choose to sponsor more than 1 hole, the same sign will appear at each hole. If you wish to sponsor holes with different signs, please submit one form for each sign.
